New Patient Registrationregister_now

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Please attend between 11.00am - 3.00pm to avoid busy times.

Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill, along with some photographic ID.

As a guide, the area covers Cleckheaton and Scholes and parts of Birkenshaw, Oakenshaw, Gomersal, Hartshead, Heckmondwike, Littletown, Liversedge and Roberttown. 

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration.

Out of area registration

From January 2015 patients may approach any GP practice, even if they live outside the practice area, to see if they will be accepted on to the patient list.

The new arrangements mean GP practices now have the option to register patients who live outside the practice area but without any obligation to provide home visits.

Consideration will be given to whether it is clinically appropriate and practical in your individual case.
